2 Benefit Items You’re eligible for benefits if you are an active full-time employee working at least 30 hours per week. Coverage goes into effect the first of the month following your date of hire. You must enroll within 30 days of your hire date. You may also enroll your eligible dependents as follows: • Your legally married spouse • Eligible children to age 26 – your natural child, lawfully adopted child (including a child placed with you for adoption but for whom the adoption is not yet final), stepchild or other child for whom you have obtained legal guardianship pursuant to a court order, until such child attains age 26 (or until such child attains age 18 in the case of legal guardianship). Listed below are the City of Hutchinson benefits available during open enrollment: • Medical • Dental • Vision • Health Savings Account • Life and AD&D • Voluntary AD&D • Short Term Disability • Employee Assistance Program • Flexible Spending Accounts • Critical Illness • Accident The open enrollment elections you make will be effective 01/01/2023 through 12/31/2023. You may only change coverage if you experience a qualifying life event. You may change your benefit elections during the year if you experience an event such as: • Marriage • Divorce or legal separation • Birth of your child or your domestic partner’s child • Death of your spouse or dependent child • Adoption of or placement for adoption of your child • Change in employment status of employee, spouse, or dependent child • Qualification by the Plan Administrator of a child support order for medical coverage • Entitlement to Medicare or Medicaid You must notify Human Resources within 30 days of a qualifying life event (or 60 days if the qualifying event is coverage lost under Medicaid or State CHIP program or if you or your dependents become eligible for a premium assistance subsidy from the state).
